I accept for use the equipment listed on this form and accept full responsibility for the care of this equipment. I understand that the sport of skiing & snowboarding and the use of this skiing & snowboarding equipment involve a risk of injury. I understand that the ski bindings are designed to reduce risk or degree of injuries from falling. The ski bindings will not release under all circumstances and is no guarantee for my safety.
I understand that the snowboarding equipment forms a part or all of a snowboard/boot/binding system which is a non release system and that its use cannot guarantee this users safety or freedom from injury while snowboarding. I agree to take this equipment as is, without any warranties, and I hereby release from any legal liability and agree to indemnify and hold harmless Green Toad Snowboard & Ski Shop, its owners, agents and employees from any and all liability for damage or injury to myself or to any person or property resulting from any negligence or other liability arising from the maintenance, selection, adjustment or use of this equipment.
I assume the full responsibility and risk of any and all such damage or injury of any kind which may result from my use of this equipment. I, the undersigned, have read, understood and agreed to the terms of the above rental and liability release agreement.
